Why should you keep track of your medical health records?

Keeping Track of your healthcare is so important. There are at least 3 important reasons to keep accurate health records.

  1. This helps you understand your own healthcare.
  2. Filling out your records with your doctor gives you a chance to discuss your current care including diagnoses and treatments.
  3. Having accurate health records in an emergency will help the physician better understand how to treat you.

There are a number of health record templates you can obtain on the internet or within programs like Microsoft Word, but I don’t think any one of them accurately captures each individual’s unique conditions.

That’s why I have created a number of different templates to help you keep accurate records. These range from your general health to specific conditions such as those with heart problems. Having specific templates can help your healthcare providers better understand your health and treat you with full knowledge of your condition.

The most important part of these templates is the what. What treatment is for what diagnosis? What medication is for what condition? Understanding the what helps you ask why. Why do I need to be on this medication and why do I need this treatment? The more you know, the more you can participate in shared-decision making with your doctor, hopefully improving your health.

I have created both full page medical health record templates and cards that you can keep in your wallet. Download whatever you feel to be applicable to your own health conditions. Make sure to ask your doctor to help you fill it out correctly.

For those with any history of heart problems, this template will be immensely valuable to you and your physician.

Remember to continually update your health records anytime you start a new treatment or have any new tests or procedures. Your doctor can help fill out the template to make sure it is accurate and that you understand your care.
